Authorization for Release of Information (from your current provider to Lifetime Insight)
To be able to address a psychiatric issue that a person may have, we thoroughly assess each person and ensure we have the diagnosis as precise as possible. In order to accomplish this, we start with a good history that contains medical information, current and previous medications and the ability to tolerate them, social and personal history, psychiatric history, substance use history, family history, and the patient’s symptoms and (sometimes family’s) concerns. This is not a history that can be done quickly if it is to be done comprehensively. Therefore, we ask that all patients fill out forms prior to coming in so that Dr. Hovav has a chance to review them before she begins her personal interview with the patient. Failure to fill out the forms in advance may cause the appointment to be canceled. Initial interviews typically last about an hour, and in some circumstances, laboratory testing may be necessary prior to beginning medication treatment. When treating older adults, involving significant others will help to define various problems and therefore reach a satisfactory and effective treatment. Receiving previous records is a crucial part of making a proper assessment, and so we urge each patient to sign an authorization for release of information. When arriving at your first appointment, please be sure to come at least 20 minutes in advance.

We are happy to offer our patients a secure patient portal to fill out all forms online, request medication refills, see laboratory results, ask the physician questions, and request appointment times. All this can be done through your phone, tablet, or desktop. In order to get started, we will register you in our system and generate a username and a temporary password. Once you are in the patient portal, make sure to fill out the legal forms and the New Patient History Form. If you are experiencing problems with the portal, call us for support prior to filling out the forms here. We recommend you only use a computer or tablet to fill out the new forms. However, once you have completed your forms, a smartphone will work great so long as you use the My Health Records app to log in. You can download the App from Google Play or iTunes!
